New Patient Forms,

It is ideal if you return the paperwork to our offices prior to your appointment either by fax or mail. If you are unable to do so please remember to bring the completed paperwork with you to our office on the day of your appointment.

We also request that you bring a copy of your insurance card(s) and, if it has not been sent to us in advance, any referral or authorization as required by your insurance.

You will receive a confirmation call two days prior to your appointment. If you are unable to keep the appointment date or time as scheduled we request a minimum of 24 hours notice to avoid a cancellation fee.

We ask that you bring with you either a list of your medications – including the dosage – or your actual pill bottles.

Lastly, please arrive 20 minutes prior to your scheduled appointment time. Again, thank you for selecting Cardiovascular Consultants to participate in your cardiology care. We look forward to having you as one of our patients.
